Registering with the surgery

When you register, it’s helpful to have your NHS number. Your NHS number is a 10 digit number that you find on any letter the NHS has sent you. For example, 444 444 4444.

To register with Castle Surgery, you can:

  1. Download and print our registration form PDF to fill in. You can then either email it to us at Castle.Surgery@wales.nhs.uk or bring to Castle Surgery reception desk directly.
  2. Collect a Registration form at the front desk and return with full information.

Please ensure you sign and date the registration which is located at the bottom of the first page of the form, (example below). Without a signature, we are unable to process your registration.


Once we have your registration with full information and signature, you will then be contacted by our Administration Team to book in for a ‘New Patient Check’. Please ensure you give us an up-to-date contact number.
